1.     What Data We Collect

AzSHA collects personal information that you provide (“Personal Data”) as well as general information about the use of its website (“Non-Personal Data”).

“Personal Data” includes any information that identifies you directly, such as your name, mailing address, email address, telephone numbers, date and/or place of birth, payment and billing information, professional certifications or licenses, bar numbers, and government-issued identification numbers. Personal Data may also include, but not be limited to, sensitive information such as racial or ethnic origin, sexual orientation, disability status, or religious preferences. The Personal Data we collect depends on how you choose to use the Services. AzSHA will not distribute Personal Data in a manner inconsistent with this Privacy Policy unless permitted or required to do so by law.

2.     How We May Use, Share and Store Your Personal Data

a.     Information Collected to Create a Member Account and Process Payment Transactions

When users of AzSHA’s website register for events or purchase/renew their membership, AzSHA will acquire certain Personal Data necessary to process the transaction and/or create an account. This may include your name, address, telephone and fax numbers, email address, and credit card number or other payment information. Payment information, such as credit card details, is only collected to process a transaction, using a secure transmission method, but it is not stored by AzSHA. Reasonable measures are taken while processing transactions to ensure that payment information is not vulnerable to theft or interception. AzSHA will not distribute your payment information in any manner that is not necessary to process a transaction. We employ the third party company called Stripe to assist AzSHA in all transactions involving payment. Stripe will have access to certain Personal Data to the extent necessary to perform those services for AzSHA. We limit the use of all such Personal Data by Stripe to the performance of such services. We take reasonable steps to obligate these third parties to handle Personal Data appropriately and confidentially. However, AzSHA cannot give absolute assurance that information intended to be maintained as private, whether transmitted via the internet or otherwise, cannot be accessed inappropriately or unlawfully by third parties.

AzSHA also requires a login and password for each member who wishes to access the members-only content online. The login and password are used to verify members’ identities and prevent unauthorized access to members’ Personal Data. Only the Board of AzSHA and its website vendor have access to this information. This information is accessed only when necessary to provide members with online account-related information, provide technical support, or diagnose or repair problems with members-only content, and is not otherwise disclosed to anyone, other than the member or his or her designee, unless permitted by law.

AzSHA may also allow users to elect to have the website remember their login information so they do not have to log in each time. If a member chooses this, a small text file called a cookie will be placed on your hard drive. The cookie tells the site that you have logged in before. It does not actually contain your password or other login information.

b.     Email Addresses and other Contact Information

When contacting AzSHA at any of the email addresses or phone numbers listed on the Website or in connection with customer service, we may ask for certain Personal Data, such as your name, email address, telephone number, or fax number. When collected for this purpose, the information is used to verify identity and to provide the assistance you are requesting.

AzSHA does not sell the email addresses (or other contact information) of users to any unaffiliated third party. Please note that even if you “opt-out” of communications about AzSHA activities, we may continue to use your email and other contact information to communicate with you in connection with your customer service request and other non-AzSHA Activities purposes.

If you are a member of AzSHA, we may list your email address and other contact information on our Membership page which is only accessible to other AzSHA members. If you do not want your contact information listed on our Membership page, please contact us at the email address provided on the Website landing page.

c.      Member Information

AzSHA offers members certain features and services through restricted access to members-only content on the Website. All members can access and update their profile, which consists of Personal Data we have collected from each member as part of their application. Member users can update some of their Personal Data and change their passwords. Member profiles are maintained for every member in AzSHA’s database. This information includes contact information such as name, firm/organization, address, telephone, fax and email address, as well as limited demographic information, such as practice area, gender, race/ethnicity, and work setting.

Limited Personal Data about members will be available to all AzSHA member users on the Membership page. Non-member users cannot view or access the Membership page. If you do not want your information listed on our Membership page, please contact us at the email address provided on the Website landing page.

d.     Discussion Forums, Blogs, Job Postings, Presentations and Other Resources

AzSHA members and other users may also contribute content to discussion forums, blogs, job postings, presentations and other resources (collectively, “User Contributions”) that may be available to other members and other users. User Contributions may or may not identify you. AzSHA may use any and all User Contributions for the purposes they were provided and to perform the Services, including but not limited to public distribution to members and non-members.

e.     Other Data Sharing

We may share your Data to fulfill the purpose for which you provided it or for which you have consented, including sharing your Data with third parties who assist us in providing the Services. We may also share your Data in response to subpoenas, court orders, requests from law enforcement, or other legal process; to establish or exercise our legal rights; to protect the personal safety of AzSHA personnel or the general public; to defend against legal claims; to a buyer or other successor of AzSHA in the event of a merger, divestiture, restructuring, reorganization, change of control, dissolution, or other sale or transfer of some or all of AzSHA assets, including as part of a bankruptcy, liquidation, or similar proceeding; or as otherwise required or permitted by applicable law.

De-identified information is information that does not contain any personal information about you or your family members or friends and for which there is not a reasonable basis to believe that it can be used to identify you. AzSHA keeps your Personal Data for as long as you are an AzSHA member, Website user, or as long as your relationship with AzSHA continues or as permitted by applicable law. We may also retain your Non-Personal Data in a depersonalized or aggregate form.

If you are a member, please note that AzSHA will keep some of your Personal Data, even after your membership with AzSHA ends in order to fulfill any final service requests; to reach out to you about AzSHA programs, events and requests to renew membership; and as reasonably necessary to comply with any legal obligations (including law enforcement requests), meet regulatory requirements, resolve disputes, maintain security, prevent fraud and abuse, enforce a user agreement, or fulfill your request to “unsubscribe” from further communication from AzSHA.


3.     Cookies and Website Usage

  • Delete all or some of your Personal Data. In some instances, AzSHA may not be able to delete your Personal Data in order to comply with applicable financial transaction laws or other laws.
  • Change or correct your Personal Data.
  • Stop using all or some of your Personal Data consistent with this Privacy Policy.
  • Give you access to the Personal Data we have about you. In most cases, you will be able to directly access this Personal Data directly on the Website.
